In-home CHRONIC CARE MANAGEMENT
Chronic Care Management for chronic conditions such as:

TELE-HEALTH VISITS
Need an appointment ASAP?
We provide telehealth visits for routine medical needs or urgent care needs via a video visit for non-life-threatening conditions such as a fever, flu, cold, sore throat and more. Your provider will discuss your symptoms, answer questions and fill a prescription, if needed, via video chat.
In-home primary care
Do any of these apply to you? Home based primary care may be right for you!
- Difficulty getting to your primary care provider with a large effort, either by you or a loved one
- Assistance needed to get to appointments or need special transportation
- Use oxygen or assistive devices such as a cane or walker
- Have difficulty walking or shortness of breath with exertion
- Have cognitive or behavioral impairment
Receive complete physical exams and routine checkups for patients ages 13 and above.
We also can complete annual physical exams including sports physicals and school physicals.
ADDITIONAL SERVICES
- Coordination for home medical equipment, including recommending, ordering and having equipment delivered to your home
- Medication review
- Referrals to community resources as needed
- Palliative care and end-of-life care
